8/2/2023 EnchantaVaughn Approved Behavior: During the home visit, it was noticed that Ben-Nita’s personality was seen cheerful, polite, stable, and relaxed. Ben-Nita presented good grooming, as evidenced by her clean appearance and clothing. Ben-Nita’s personality was observed as cheerful, cordial, polite, and friendly. Ben-Nita displayed appropriate affect and easy-going behaviors as she engaged in an appropriate manner with QMHP-C. Behavior: Ben-Nita displayed withdrawn behaviors with QMHP-C at times within the session as she was starring off at the walls not listening to the review of the session. Intervention: QMHP-C greeted Ben-Nita and inquired how her day was going. QMHP-C actively listened as information was shared by Ben-Nita. QMHP-C discussed with mom about Ben-Nita’s involvement during their session and how much improvement is noticed so far since the first initial session. QMHP-C modeled how to show Ben-Nita more support. QMHP-C discussed ways to increase more positive coping mechanisms and how to display more positive behaviors. Intervention: QMHP-C had one on one with Ben-Nita. QMHP-C explored positive behavior options she was exhibiting with Ben-Nita to support her willingness to continue to make good decisions and display appropriate behaviors in the home. QMHP-C reviewed Ben-Nita goals and objectives one more time to make sure Ben-Nita recognizes areas of improvement and progress. QMHP-C talked to Ben-Nita about her confidence levels and how they can affect her day and self-esteem. QMHP-C encouraged Ben-Nita to focus on her strengths instead of her weaknesses and failures. QMHP-C coached Ben-Nita to acknowledge and list her strengths, resilience and the positive outcomes that often occur when she makes informed decisions opposed to impulsive ones. QMHP-C commended Ben-Nita for her positive communication modeled in session. Response: Ben-Nita had one on one with QMHP-C. Ben-Nita verbalized positive feelings about evening and morning. Ben-Nita said she was feeling good and plans to have a good day. Ben-Nita was receptive to the education and training. Ben-Nita gained knowledge about following directions the first time and being a good listener. Ben-Nita demonstrated good social skills and ability to get along with others. Ben-Nita expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. Ben-Nita responded well to feedback, interacting well with everyone in the home. Response: Ben-Nita responded well to the positive reinforcement provided by QMHP-C. Ben-Nita said thank you and shared feedback of her own to add to the discussion. Ben-Nita displayed management of individualized behaviors. Ben-Nita has not overreacted with anger to minor frustrations or irritants. Ben-Nita responded in a pleasant manner acknowledging the positive reinforcement that QMHP-C provided. Ben-Nita is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation with peers, and waiting for her turn to talk.
